WISHES
The Five Wishes document is a valuable tool
that assists you in recording decisions about end-of-life issues. By
completing this document you will make your wishes known in regards
to who is your durable power of attorney for health care issues,
what kind of medical treatment you do or don't want if you were
seriously ill or injured, and how you want to be treated and cared
for when you can't take care of yourself. You are also able to
record information about how you want to be remembered. By
completing the Five Wishes document you are sharing your wants and
needs in a way that is legally binding.

Dignity Memorial® Funeral Service Plans
With four plans to choose from, available
either pre-need or at time of need, Dignity Memorial® Plans provide
value in the form of simplicity of decision-making, choice and
affordability. All plans include use of a hearse, a full traditional
funeral service and a committal service.
Designed to offer a range of choices, our
four plans - Platinum, Heritage, Value, and Tribute - identify
specific options including the service, the casket, the memorial
package, flowers, outer burial container and family support benefit.
Families can modify the plans as necessary to create a personalized
package specific to their needs. |
